JOB RESPONSIBILITIES

We are looking for someone who is flexible, creative, and has a problem-solving ability. The position is a full-time temporary position (100%), placed in the Social Support unit within Support and Housing.

Social Support is the municipality's housing support service within social psychiatry/co-occurring disorders. The work involves Monday to Friday, daytime work.

The operations work in a team-based and cross-professional manner, and the role includes case management.

In this role, you are expected to:

• Follow the routines, policies, and guidelines that are current in Hammarö municipality
• Follow the assignment established by the authority
• Support individuals in their daily lives according to the assignment
• Follow up on interventions, goals, and implementation plans
• Use methods such as RACT to map and follow clients' needs.
• Gather and provide relevant information to and from the right function, which means identifying who should receive the information or which function you should turn to for a correct answer to your question; these functions may include occupational therapists, nurses, health centers, or managers.
• Work with internal and external collaboration

QUALIFICATIONS

We are looking for someone who has:

• Higher vocational education in social pedagogy, as a treatment assistant, or other education deemed equivalent by the employer.
• Experience working with individuals with complex issues/co-occurring disorders
• Experience working with individuals with challenging behavior
• Experience in low-arousal approaches
• Experience in clarifying pedagogy.
• Experience in documentation and record-keeping
• Good computer skills and knowledge of IBIC, and can master the Swedish language in speech and writing.

Meritorious:

• Training in RACT/ACT/Case Management
• Training in clarifying pedagogy
• Training in conflict and crisis management or similar violence and threat training
• Knowledge of the new Social Services Act

You should be confident in your professional role and have the ability to work independently. Collaboration with other actors, both internally and externally, is also part of the job, which places high demands on cooperation skills.

Personal suitability and interest in working with the target group are important. The job requires high sensitivity, personal responsibility, and strong communication skills. The work involves walking and, in some cases, exercising with clients.

A driving license for manual transmission is required, as well as the ability to cycle for work purposes.
